4. Nature of Quotes & Subsidy Estimates

Final eligibility for Advance Premium Tax Credits (APTC) and Cost-Sharing Reductions (CSR) is determined exclusively by the Federally Facilitated Marketplace (HealthCare.gov) or state health exchanges upon formal verification of your identity, household tax filing status, and income documentation. 5. User Obligations & Truthful Representations

• You understand that misrepresenting household income or family size on an ACA application may lead to subsidy clawbacks during annual IRS tax reconciliation (Form 8962) or policy cancellation by the carrier.

6. Insurance Carrier Contracts & Underwriting

Prime Health Global is an independent brokerage agency and is not an insurance underwriter or risk-bearing health plan. When you purchase an insurance policy, your coverage agreement constitutes a legal contract solely between you (the policyholder) and the underwriting insurance company (e.g., UnitedHealthcare, Blue Cross Blue Shield, Aetna, Cigna, Humana, Kaiser Permanente, Oscar, Molina, or Ambetter).

All policy terms, conditions, exclusions, co-payment obligations, deductible thresholds, prior-authorization rules, and prescription formularies are governed exclusively by the carrier’s official Evidence of Coverage (EOC), Summary of Benefits and Coverage (SBC), and policy rider contracts. In the event of any conflict between materials on our website and official carrier policy documents, the carrier’s contract controls.

12. Governing Law, Dispute Resolution & Contact

These Terms shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law principles. Any legal suit, action, or proceeding arising out of or related to these Terms shall be instituted exclusively in the federal or state courts located in Miami-Dade County or Hillsborough County, Florida.
